Lee Vogel, MD
Dr. Vogel joined our program in 1990. She is a Minnesota native and a graduate of the University of Minnesota Medical School. She completed her family practice residency and a fellowship in geriatrics at the University of California, San Diego. She was also on the faculty for two years at UCSD before coming to Appleton. Besides her broad base of clinical family medicine knowledge, Dr. Vogel has a strong interest in teaching geriatrics to residents in the office and nursing home settings. Other areas of expertise include medical ethics and health care promotion.

Dr. Menzel grew up in Shawano, Wisconsin, a small lakeside town about 40 miles north of Appleton. She received a Bachelor of Science degree in psychology from the University of Wisconsin – Madison, and attended Mayo Medical School prior to completing her training with the University of Wisconsin Fox Valley Family Medicine Residency. Subsequently, she joined the faculty of the Fox Valley Family Medicine Residency program. Dr. Menzel thoroughly enjoys her current role as Program Director while continuing to teach and practice the full spectrum of Family Medicine with special interest in prenatal care/obstetrics, adolescent medicine, geriatrics and health advocacy and education. Outside of work, she cherishes time with her family, especially cheering for the Packers and Badgers and enjoying the great outdoors.

Megan Everson, MD
Megan Everson, MD
Dr. Everson grew up in Viroqua, Wisconsin, a small town in the southwest portion of the state. She received her Bachelor of Arts degree in biology from Luther College in Decorah, Iowa and subsequently attended the University of Wisconsin School of Medicine and Public Health as a member of the WARM (Wisconsin Academy for Rural Medicine) program prior to completing her medical training with the Fox Valley Family Medicine Residency program. Dr. Everson enjoys teaching residents and helping her patients achieve their health goals, with additional special interests in prenatal and obstetric care, geriatrics, transgender care, integrative medicine, and dermatology. Outside of work, she enjoys baking and spending time with her family.

Alice M Boshoven, MD
After earning a degree in Biology with a minor in Biological Anthropology from the University of Michigan, Alice worked for a year as a research assistant at the University of Michigan Cancer Center. While attending medical school at the University of Wisconsin School of Medicine and Public Health, she volunteered in a student run free clinic serving an uninsured population and tutored other medical students. She joined Mosaic Family Health in 2013 and her special interests include Medication-assisted treatment for opioid use disorder, pediatrics, geriatrics, prenatal care, natural family planning and natural procreative technology, dermatology, hospital medicine, and teaching.
Dr. Boshoven enjoys running, playing tennis, spending time with her family, and being involved at her church.

Kathryn Jacobe, MD, IBCLC, FAAFP
Dr. Jacobe graduated from our Fox Valley Family Medicine Residency Program in 2011 and joined the faculty that year. She grew up in Green Bay, and attended the University of Wisconsin Madison for both her undergraduate studies in English and Biology, and her Medical Degree. Dr. Jacobe enjoys her role as full scope faculty with obstetrics, particularly the opportunity to help residents grow over their 3 years here into capable confident physicians! Dr. Jacobe has special interest in pediatrics, including a special focus clinic for mother-baby dyads who are struggling with feeding problems. In 2019 Dr. Jacobe obtained certification as an internationally board certified lactation consultant, and enjoys sharing her knowledge in this area with patients and residents alike. When not at work, she enjoys time with her husband and four children, camping, skiing, crafting, cooking, gardening, and snuggling for movie nights.

Allen R Last, MD, MPH
Dr. Last grew up in the Fox Valley and was previously a faculty member and program director at the Racine Family Medicine Residency program, prior to coming back to Appleton. He enjoys caring for people of all ages – from newborns to the elderly. Dr. Last is the author of several publications, and his clinical interests include Medication-assisted treatment for opioid use disorder, sports medicine and dermatology.
When not working, he enjoys spending time with his family, building furniture and participating in various outdoor activities.

Deborah Schultz, MD
Dr. Schultz coordinates geriatrics services at Mosaic Family Health alongside Dr. Lee Vogel, and provides patient care in the long-term care setting. She was the volunteer medical director of Calumet County Hospice for twenty years until handing this off to Dr. Dellaria, a FVFMR graduate, now working in Chilton. Her areas of expertise and interest are in geriatrics, hospice and palliative care, medical humanities and the management of chronic pain and opioid use disorder.

Dr. Verhagen grew up in the Fox Valley. She earned her medical degree from Chicago College of Osteopathic Medicine and completed her residency at the Fox Valley Family Medicine Residency Program. Her special interests include preventative medicine, health equity, gender affirming care, osteopathic manipulative treatment, and geriatrics. She is board certified by the ABFM and AOBFP with certification in OMT completed.
She is a graduate of the Fox Valley Family Medicine Residency Program and is thrilled to have continued her journey with Mosaic Family Health as a faculty member and as the Osteopathic Site Director. She loves that she gets to continue to provide care for her community while cherishing the lifelong learning and growth that comes with being a family physician.

Jackie joined Mosaic Family Health in February of 2019. She received her Bachelor of Science degree from UW-Oshkosh in Psychology. Throughout her career of over 26 years in the Fox Cities she has been involved in positions of leadership/supervision on a local and regional levels. Jackie has had the opportunity to work with people of all ages and needs throughout her career and very much values being a resource and advocate for people in need. She truly recognizes the importance of and supports the blending of physical and mental health care with social services for people in our community.

Job title: Behavioral Health Counselor
Teri is a State Certified Social Worker and Licensed Professional Counselor. She received her Bachelor of Science degree in Psychology from the University of Wisconsin La Crosse and her Master’s Degree in Counseling from Lakeland University. Teri completed her 3000 post masters, clinical hours under the direction of a psychiatrist with emphasis on psychotherapeutic care.
Teri has worked as a State Certified Social Worker or a Licensed Professional Counselor in the Fox Valley since 1990. Her work has spanned all generations, including work as a child welfare social worker, a State of Wisconsin treatment foster care administrator, a juvenile justice mentoring coordinator, manager of a non-profit for the criminal justice system, and 12 years working in End of Life Care as both a clinician and in a leadership role. Through her career experiences, Teri has found that her passion lies in supporting and empowering individuals to achieve their optimal level of mental health to be able to live and engage fully in their best life.

Dr. Michael Werner is appointed as a Clinical Assistant Professor with the Fox Valley Family Medicine Residency Program and provides hospital call coverage and staffing services as well as teaching GI procedures to residents. His patient care practice is with the Primary Care Associates.
Dr. Werner is a 2006 graduate of the Fox Valley Family Medicine Residency Program and is Board Certified in Family Medicine. He received a BS in Bacteriology from the University of Wisconsin – Madison and his medical degree from the Medical College of Wisconsin in Milwaukee. Professional interests include diabetes, geriatrics, obstetrics, sports medicine and office procedures. He is a member of the American Academy of Family Physicians, Wisconsin Academy of Family Physicians and the Wisconsin Medical Society. Dr. Werner enjoys time with his family, enjoys sports, travel, history and reading.

Dr. Wendy Schroeder is appointed as a Clinical Assistant Professor and provides staffing services for resident physicians with the Fox Valley Family Medicine Residency Program.
Dr. Schroeder is a 2009 graduate of the UW Fox Valley Family Medicine Residency Program and is a native of Appleton. She completed medical school at Des Moines University Osteopathic Medical Center, after earning a BS at the University of Wisconsin in La Crosse. Dr. Schroeder’s professional interests include Osteopathic Manipulative Medicine, Athletic Training and Community Medicine. She is an avid Packer fan and enjoys volleyball, camping, fishing, and hunting, reading mystery novels and spending time with her family.
